Initial Consultation

Your first appointment includes diagnosis explanation and treatment options. Treatment may happen same-day, but complex cases need a separate evaluation and appointment for treatment.

Please provide the following information before or at the time of consultation:

• Patient Registration
• Consent
• Patient Health History
• Consent
• Financial Policies
• Referral paperwork, x-rays, CT scans, MRI films, and sleep studies (if applicable)
• Current medical prescriptions
• If you have medical insurance, bring the necessary completed forms and your insurance card. 

IMPORTANT: A parent or guardian must accompany all patients under 18.

Financial Policy

Dental appliances used to treat sleep apnea typically are NOT covered by dental insurance. Often times though, medical insurance and medicare WILL provide coverage. Our office will work with you to determine if you have medical benefits, and facilitate filing your insurance claim. 

We also offer financing through CareCredit. 

We accept Visa, MasterCard and Discover. and deliver the finest care at the most reasonable cost to every patient.

Please remember patients are fully responsible for all fees charged regardless of insurance coverage.

Most insurance companies will respond within four to six weeks. Any remaining balance after insurance is the responsibility of the patient.
